This variant is considered likely pathogenic. mRNA analysis has demonstrated abnormal mRNA splicing occurs [Myriad internal data]. -
Hereditary cancer-predisposing syndrome Pathogenic:1
The c.7007+5G>C intronic variant results from a G to C substitution 5 nucleotides after coding exon 12 in the BRCA2 gene. This nucleotide position is well conserved in available vertebrate species. In silico splice site analysis predicts that this alteration may weaken the native splice donor site. RNA studies have demonstrated that this alteration results in abnormal splicing in the set of samples tested (Ambry internal data). Several close-match alterations that are expected to have a similar splicing profile, BRCA2 c.7007G>A and BRCA2 c.7007G>C, have been observed in multiple individuals with Fanconi Anemia (Meng L et al. JAMA Pediatr. 2017 12;171(12):e173438; Barber LM et al. Br. J. Haematol. 2005 Sep;130:796-7). Based on the majority of available evidence to date, this variant is likely to be pathogenic. However, because several variants in this region with similar splicing profiles were identified in individuals with Fanconi Anemia, this variant may be hypomorphic and thus carriers of this variant and their families may present with reduced risks, and not with the typical clinical characteristics of a high-risk pathogenic BRCA2 alteration. As risk estimates are unknown at this time, clinical correlation is advised. -
Hereditary breast ovarian cancer syndrome Pathogenic:1
This variant has not been reported in the literature in individuals affected with BRCA2-related conditions. This variant is not present in population databases (gnomAD no frequency). This sequence change falls in intron 13 of the BRCA2 gene. It does not directly change the encoded amino acid sequence of the BRCA2 protein. RNA analysis indicates that this variant induces altered splicing and may result in an absent or disrupted protein product. ClinVar contains an entry for this variant (Variation ID: 52240). For these reasons, this variant has been classified as Pathogenic. This variant disrupts the c.7007+5G nucleotide in the BRCA2 gene. Other variant(s) that disrupt this nucleotide have been determined to be pathogenic (PMID: 9536098, 17576681, 30254663; Invitae). This suggests that this nucleotide is clinically significant, and that variants that disrupt this position are likely to be disease-causing. Variants that disrupt the consensus splice site are a relatively common cause of aberrant splicing (PMID: 17576681, 9536098). Studies have shown that this variant results in skipping of exon 13 and skipping of exons 12 and 13 and introduces a premature termination codon (Invitae). The resulting mRNA is expected to undergo nonsense-mediated decay. Based on a multifactorial likelihood algorithm using genetic, in silico, and/or statistical data, this variant has been determined to have a high probability of being pathogenic (PMID: 31131967). -
