SUCLA2 (HGNC:11448): (succinate-CoA ligase ADP-forming subunit beta) Succinyl-CoA synthetase (SCS) is a mitochondrial matrix enzyme that acts as a heterodimer, being composed of an invariant alpha subunit and a substrate-specific beta subunit. The protein encoded by this gene is an ATP-specific SCS beta subunit that dimerizes with the SCS alpha subunit to form SCS-A, an essential component of the tricarboxylic acid cycle. SCS-A hydrolyzes ATP to convert succinate to succinyl-CoA. Defects in this gene are a cause of myopathic mitochondrial DNA depletion syndrome. Variant summary: SUCLA2 c.850C>T (p.Arg284Cys) results in a non-conservative amino acid change located in the ATP-grasp fold domain (IPR011761) of the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 3.2e-05 in 250822 control chromosomes (gnomAD). c.850C>T has been reported in the literature in multiple individuals affected with features of Mitochondrial DNA Depletion Syndrome, Encephalomyopathic Form With Methylmalonic Aciduria (e.g. Carrozzo_2007, de Castro_2020). These data indicate that the variant is very likely to be associated with disease. The following publications have been ascertained in the context of this evaluation (PMID: 17301081, 32718099). ClinVar contains an entry for this variant (Variation ID: 5978). Based on the evidence outlined above, the variant was classified as pathogenic. -

This missense change has been observed in individuals with mitochondrial DNA depletion syndrome (PMID: 17301081, 28749033, 30315573, 32718099). ClinVar contains an entry for this variant (Variation ID: 5978). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt SUCLA2 protein function with a positive predictive value of 80%. Studies have shown that this missense change alters SUCLA2 gene expression (PMID: 17301081).
