Variant summary
Our verdict is Likely pathogenic. The variant received 8 ACMG points: 8P and 0B. PM2PP2PP3_StrongPP5
The NM_000053.4(ATP7B):c.2267C>T(p.Ala756Val) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.0000124 in 1,614,080 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Synonymous variant affecting the same amino acid position (i.e. A756A) has been classified as Likely benign.

This missense variant replaces alanine with valine at codon 756 of the ATP7B protein. Computational prediction suggests that this variant may have deleterious impact on protein structure and function (internally defined REVEL score threshold >= 0.7, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been observed in two individuals affected with autosomal recessive Wilson disease in the compound heterozygous state (PMID: 16423615, 35470480). It has also been observed in double homozygosity with a pathogenic ATP7B c.2297C>T p.Thr766Met variant (ClinVar Variation ID: 633064) in three siblings with Wilson disease born to unaffected consanguineous parents, both of whom carried p.Ala756Val and p.Thr766Met variants in cis (PMID: 31059521). This variant has been identified in 3/249586 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
