Variant summary
Our verdict is Pathogenic. Variant got 14 ACMG points: 14P and 0B. PM2PM5PP3_ModeratePP5_Very_Strong
The NM_000153.4(GALC):c.388G>A(p.Glu130Lys) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.00000434 in 1,613,708 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Likely pathogenic (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. E130G) has been classified as Likely pathogenic.

Pathogenic, criteria provided, single submitter | clinical testing | Women's Health and Genetics/Laboratory Corporation of America, LabCorp | Feb 17, 2022 | Variant summary: GALC c.388G>A (p.Glu130Lys) results in a conservative amino acid change in the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 8e-06 in 249416 control chromosomes (gnomAD). c.388G>A has been reported in the literature in a homozygous and multiple compound heterozygous individuals affected with Krabbe Disease (example: Guenzel_2020, Tappino_2010 and Lissens_2007). These data indicate that the variant is very likely to be associated with disease. Guenzel_2020 demonstrated the homozygous individual in this study had significantly reduced GALC activity in WBC. No clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014. Based on the evidence outlined above, the variant was classified as pathogenic. - |

Pathogenic, criteria provided, single submitter | clinical testing | Labcorp Genetics (formerly Invitae), Labcorp | Jan 20, 2024 | This sequence change replaces glutamic acid, which is acidic and polar, with lysine, which is basic and polar, at codon 130 of the GALC protein (p.Glu130Lys). This variant is present in population databases (rs374635469, gnomAD 0.002%). This missense change has been observed in individual(s) with Krabbe disease (PMID: 17579360, 20886637). This variant is also known as c.340G>A Glu114Lys. ClinVar contains an entry for this variant (Variation ID: 188817). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt GALC protein function with a positive predictive value of 95%. Experimental studies have shown that this missense change affects GALC function (PMID: 23620143, 27638593). For these reasons, this variant has been classified as Pathogenic. - |

Likely pathogenic, criteria provided, single submitter | clinical testing | GeneDx | Oct 20, 2022 | Published functional studies demonstrate loss-of-function (Potter et al., 2013; Saavedra-Matiz et al., 2016); Not observed at significant frequency in large population cohorts (gnomAD); In silico analysis supports that this missense variant has a deleterious effect on protein structure/function; Also known as E114K using alternate nomenclature; This variant is associated with the following publications: (PMID: 23620143, 27638593, 17579360, 20886637, 31589614, 35013804) - |
