ABC proteins transport various molecules across extra- and intra-cellular membranes. ABC genes are divided into seven distinct subfamilies (ABC1, MDR/TAP, MRP, ALD, OABP, GCN20, White). The encoded protein, a member of the MRP subfamily, is involved in multi-drug resistance. Mutations in this gene cause pseudoxanthoma elasticum. Variant summary: ABCC6 c.2420G>A (p.Arg807Gln) results in a conservative amino acid change located in the ATP-binding domain (IPR003439) of the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 2.4e-05 in 251326 control chromosomes (gnomAD). c.2420G>A has been reported in the literature in multiple individuals affected with Pseudoxanthoma Elasticum (e.g. Miksch_2005, Garcia-Fernandez_2008, Kringen_2015, Verschuere_2021). Other missense variants affecting the same residue (R807W, R807G) have been also reported in patients affected with Pseudoxanthoma Elasticum (HGMD). These data indicate that the variant is very likely to be associated with disease. Three submitters have submitted clinical-significance assessments for this variant to ClinVar after 2014 and classified the variant as pathogenic (n=2) and likely pathogenic (n=1). Based on the evidence outlined above, the variant was classified as pathogenic. -

This sequence change replaces arginine, which is basic and polar, with glutamine, which is neutral and polar, at codon 807 of the ABCC6 protein (p.Arg807Gln). This variant is present in population databases (rs72653794, gnomAD 0.005%). This missense change has been observed in individuals with pseudoxanthoma elasticum (PMID: 16086317, 18513494, 32873932). ClinVar contains an entry for this variant (Variation ID: 381638). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt ABCC6 protein function with a positive predictive value of 95%. For these reasons, this variant has been classified as Pathogenic. -

The R807Q likely pathogenic variant has been reported in the compound heterozgyous state with another variant in patients with PXE (Miksch et al., 2005; Garcia-Fernandez et al., 2008). The R807Q variant was not observed at any significant frequency in approximately 6500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project. The R807Q variant is a semi-conservative amino acid substitution, which may impact secondary protein structure as these residues differ in some properties. This substitution occurs at a position that is conserved across species, and in silico analysis predicts this variant is probably damaging to the protein structure/function. Missense variants at the same residue (R807W) and in nearby residues (V810M, T811M) have been reported in the Human Gene Mutation Database in association with PXE (Stenson et al., 2014), supporting the functional importance of this region of the protein. Therefore, this variant is likely pathogenic; however, the possibility that it is benign cannot be excluded. -
