Variant summary
Our verdict is Likely benign. The variant received -2 ACMG points: 3P and 5B. PM5PP3BP6BS2
The NM_000548.5(TSC2):c.2357G>A(p.Arg786His) variant causes a missense, splice region change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.0000211 in 1,610,980 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. 3/3 splice prediction tools predict no significant impact on normal splicing. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. R786C) has been classified as Likely benign.

In silico analysis supports that this missense variant does not alter protein structure/function; Observed in a patient with renal cell carcinoma (PMID: 35441217); This variant is associated with the following publications: (PMID: 35441217) -
Hereditary cancer-predisposing syndrome Uncertain:1
The p.R786H variant (also known as c.2357G>A), located in coding exon 21 of the TSC2 gene, results from a G to A substitution at nucleotide position 2357. The arginine at codon 786 is replaced by histidine, an amino acid with highly similar properties. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear. -
