Variant summary: PALB2 c.3436C>A (p.Gln1146Lys) results in a conservative amino acid change in the WD40 domain (IPR031920); this domain is involved binding to the N terminus of BRCA2 (InterPro). Five of five in-silico tools predict a benign effect of the variant on protein function. The variant was absent in 246248 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. To our knowledge, no occurrence of c.3436C>A in individuals affected with Hereditary Breast and Ovarian Cancer and no experimental evidence demonstrating its impact on protein function have been reported. Two clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation, and both classified the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as uncertain significance. -

The p.Q1146K variant (also known as c.3436C>A), located in coding exon 13 of the PALB2 gene, results from a C to A substitution at nucleotide position 3436. The glutamine at codon 1146 is replaced by lysine, an amino acid with similar properties. This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

This sequence change replaces glutamine, which is neutral and polar, with lysine, which is basic and polar, at codon 1146 of the PALB2 protein (p.Gln1146Lys). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with PALB2-related conditions. ClinVar contains an entry for this variant (Variation ID: 245984). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be tolerated. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
