Hereditary cancer-predisposing syndrome Uncertain:3
The p.S1065C variant (also known as c.3194C>G), located in coding exon 11 of the PALB2 gene, results from a C to G substitution at nucleotide position 3194. The serine at codon 1065 is replaced by cysteine, an amino acid with dissimilar properties. This variant has been identified in breast cancer cohorts (Weitzel JN et al. Cancer, 2019 Aug;125:2829-2836; Guindalini RSC et al. Sci Rep, 2022 Mar;12:4190). Internal structural analysis indicates this variant in structurally disruptive (Oliver AW et al 2009 EMBO Rep. 2009 Sep; 10(9): 990–996; Siaud N et al, PLoS Genet. 2011 Dec; 7(12):e1002409; Ambry internal data). This amino acid position is well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this variant remains unclear.
This missense variant replaces serine with cysteine at codon 1065 of the PALB2 protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). Splice site prediction tools suggest that this variant may not impact RNA splicing. To our knowledge, functional studies have not been performed for this variant. This variant has not been reported in individuals affected with hereditary cancer in the literature. This variant has been identified in 1/31404 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.
Familial cancer of breast Uncertain:2
This sequence change replaces serine, which is neutral and polar, with cysteine, which is neutral and slightly polar, at codon 1065 of the PALB2 protein (p.Ser1065Cys). This variant is present in population databases (no rsID available, gnomAD no frequency). This missense change has been observed in individual(s) with breast cancer (PMID: 35264596). ClinVar contains an entry for this variant (Variation ID: 480249). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt PALB2 protein function with a positive predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
