Variant summary
Our verdict is Pathogenic. The variant received 14 ACMG points: 14P and 0B. PS1_ModeratePM1PM2PM5PP3_StrongPP5_Moderate
The NM_145239.3(PRRT2):c.913G>C(p.Gly305Arg) variant causes a missense change. The variant allele was found at a frequency of 0.000000686 in 1,456,674 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Pathogenic (★). Another nucleotide change resulting in the same amino acid substitution has been previously reported as Likely pathogenic in ClinVar. Another variant affecting the same amino acid position, but resulting in a different missense (i.e. G305E) has been classified as Likely pathogenic.

This sequence change replaces glycine, which is neutral and non-polar, with arginine, which is basic and polar, at codon 305 of the PRRT2 protein (p.Gly305Arg). This variant is present in population databases (no rsID available, gnomAD 0.003%). A different variant (c.913G>A) giving rise to the same protein effect has been determined to be pathogenic (PMID: 22209761, 22895590, 30392205). This suggests that this variant is also likely to be causative of disease. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt PRRT2 protein function with a positive predictive value of 95%. For these reasons, this variant has been classified as Pathogenic.
