The isoleucine at codon 96 is replaced by phenylalanine, an amino acid with highly similar properties. This alteration has been identified in a Chinese woman diagnosed with breast cancer (Xie Y et al. Clin. Genet., 2018 Jan;93:41-51). This variant was reported in 4/60,466 breast cancer cases and in 5/53,461 controls (Dorling et al. N Engl J Med 2021 02;384:428-439). This amino acid position is poorly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Three of five in-silico tools predict a benign effect of the variant on protein function. The variant was absent in 251330 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. The variant, c.286A>T, has been reported in the literature in an individual affected with Breast Cancer (Xie_2018). However, a recent large-scale Breast Cancer study reported the variant in 4/60466 cases and 5/53461 controls, i.e. with a calculated odds ratio of 0.7, which suggests that this variant is not associated with a risk for Breast Cancer (Dorling_2021). To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Three clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation.
