Genes affected
CDH1 (HGNC:1748): (cadherin 1) This gene encodes a classical cadherin of the cadherin superfamily. Alternative splicing results in multiple transcript variants, at least one of which encodes a preproprotein that is proteolytically processed to generate the mature glycoprotein. This calcium-dependent cell-cell adhesion protein is comprised of five extracellular cadherin repeats, a transmembrane region and a highly conserved cytoplasmic tail. Mutations in this gene are correlated with gastric, breast, colorectal, thyroid and ovarian cancer. Loss of function of this gene is thought to contribute to cancer progression by increasing proliferation, invasion, and/or metastasis. The ectodomain of this protein mediates bacterial adhesion to mammalian cells and the cytoplasmic domain is required for internalization. This gene is present in a gene cluster with other members of the cadherin family on chromosome 16. [provided by RefSeq, Nov 2015]

Uncertain significance, no assertion criteria provided | clinical testing | Department of Pathology and Laboratory Medicine, Sinai Health System | - | The CDH1 p.Arg545Trp variant was not identified in the literature. The variant was identified in dbSNP (ID: rs863224727) as “With Uncertain significance allele” and ClinVar (as uncertain significance by Invitae and one other submitter). The variant was identified in control databases in 1 of 246266 chromosomes at a frequency of 0.000004 (Genome Aggregation Database Feb 27, 2017). The variant was observed in the South Asian population in 1 of 30778 chromosomes (freq: 0.000032), while the variant was not observed in the African, Other, Latino, European Non-Finnish, Ashkenazi Jewish, East Asian, or European Finnish populations. The variant was identified by our laboratory with a co-occurring, pathogenic BRCA2 variant (c.5946del, p.Ser1982Argfs*22), increasing the likelihood that the CDH1 p.Arg545Trp variant does not have clinical significance. The p.Arg545 residue is conserved in mammals but not in more distantly related organisms and four out of five computational analyses (PolyPhen-2, SIFT, AlignGVGD, BLOSUM, MutationTaster) suggest that the Trp variant may impact the protein; however, this information is not predictive enough to assume pathogenicity. The variant occurs outside of the splicing consensus sequence and in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer) do not predict a difference in splicing. In summary, based on the above information, the clinical significance of this variant cannot be determined with certainty at this time. This variant is classified as a variant of uncertain significance. - |
Uncertain significance, criteria provided, single submitter | clinical testing | GeneDx | Oct 28, 2019 | Not observed at a significant frequency in large population cohorts (Lek 2016); In silico analysis, which includes protein predictors and evolutionary conservation, supports a deleterious effect; Observed in individuals with breast cancer (Momozawa 2018); This variant is associated with the following publications: (PMID: 30287823) - |
Hereditary cancer-predisposing syndrome Uncertain:2
Uncertain significance, criteria provided, single submitter | clinical testing | Ambry Genetics | Nov 07, 2022 | The p.R545W variant (also known as c.1633C>T), located in coding exon 11 of the CDH1 gene, results from a C to T substitution at nucleotide position 1633. The arginine at codon 545 is replaced by tryptophan, an amino acid with dissimilar properties. This alteration was observed in 1/7051 unselected female breast cancer patients and was not detected in 11241 female controls of Japanese ancestry (Momozawa Y et al. Nat Commun, 2018 10;9:4083). This alteration was also identified in 1/12503 unselected Japanese colorectal cancer patients and in 0/23705 controls (Fujita M et al. Clin Gastroenterol Hepatol, 2022 09;20:2132-2141.e9). This alteration has been detected in a patient with breast cancer at age 44; however, they were also found to be positive for a pathogenic mutation in the BRCA2 gene (Lerner-Ellis J et al. J Cancer Res Clin Oncol, 2021 Mar;147:871-879). This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. - |

Uncertain significance, no assertion criteria provided | clinical testing | MVZ Praenatalmedizin und Genetik Nuernberg | Nov 01, 2017 | This rare/private variant (gnomAD: 1/246266 alleles) was not found in databases and literature, only 1 COSMIC-entry. Multiple in silico analyses are contradictory. Therefore we rate this variant as Variant of unknown significance (VUS) - |

Hereditary diffuse gastric adenocarcinoma Uncertain:1
Uncertain significance, criteria provided, single submitter | clinical testing | Labcorp Genetics (formerly Invitae), Labcorp | Nov 23, 2022 | This variant is present in population databases (no rsID available, gnomAD 0.003%). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Probably Damaging"; Align-GVGD: "Class C0"). ClinVar contains an entry for this variant (Variation ID: 446218). This missense change has been observed in individual(s) with breast cancer (PMID: 30287823). This sequence change replaces arginine, which is basic and polar, with tryptophan, which is neutral and slightly polar, at codon 545 of the CDH1 protein (p.Arg545Trp). - |
