Variant summary
Our verdict is Likely pathogenic. Variant got 6 ACMG points: 6P and 0B. PM2PP2PP3_ModeratePP5
The NM_001042492.3(NF1):c.6295C>T(p.His2099Tyr) variant causes a missense change involving the alteration of a conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars).

The NF1 c.6295C>T; p.His2099Tyr variant (rs1060500382), to our knowledge, is not reported in the medical literature but is reported in ClinVar (Variation ID: 404611). This variant is absent from the Genome Aggregation Database (v2.1.1), indicating it is not a common polymorphism. Computational analyses predict that this variant is deleterious (REVEL: 0.727). However, given the lack of clinical and functional data, the significance of this variant is uncertain at this time. -
Has not been previously published as pathogenic or benign to our knowledge; Not observed in large population cohorts (gnomAD); In silico analysis supports that this missense variant has a deleterious effect on protein structure/function -
Neurofibromatosis, type 1 Uncertain:2
This sequence change replaces histidine, which is basic and polar, with tyrosine, which is neutral and polar, at codon 2078 of the NF1 protein (p.His2078Tyr). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with NF1-related conditions. ClinVar contains an entry for this variant (Variation ID: 404611). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt NF1 protein function with a positive predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

Hereditary cancer-predisposing syndrome;CN230736:Cardiovascular phenotype Pathogenic:1
The p.H2078Y variant (also known as c.6232C>T), located in coding exon 41 of the NF1 gene, results from a C to T substitution at nucleotide position 6232. The histidine at codon 2078 is replaced by tyrosine, an amino acid with similar properties. This variant has been observed in at least one individual with a personal and/or family history that is consistent with Neurofibromatosis type 1 (External communication). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the majority of available evidence to date, this variant is likely to be pathogenic. -
