This variant has been observed in individual(s) with clinical features of NF1-related conditions (PMID: 12808981, 18546366, 29673180; Invitae). In at least one individual the variant was observed to be de novo. For these reasons, this variant has been classified as Pathogenic. Variants that disrupt the consensus splice site are a relatively common cause of aberrant splicing (PMID: 17576681, 9536098). Studies have shown that this variant results in skipping of exon 44 (also known as exon 36) and introduces a premature termination codon (PMID: 12807981). The resulting mRNA is expected to undergo nonsense-mediated decay. ClinVar contains an entry for this variant (Variation ID: 457805). This variant is also known as IVS36+3G>A. This variant is not present in population databases (gnomAD no frequency). This sequence change falls in intron 44 of the NF1 gene. It does not directly change the encoded amino acid sequence of the NF1 protein. RNA analysis indicates that this variant induces altered splicing and may result in an absent or disrupted protein product.

The NF1 c.6756+3A>G variant has been reported in the published literature in individuals affected with neurofibromatosis type 1 (PMID: 29673180 (2018), 18546366 (2008)). Functional studies demonstrate abnormal splicing (PMID: 12807981 (2003)). This variant has not been reported in large, multi-ethnic general populations (Genome Aggregation Database, http://gnomad.broadinstitute.org). Analysis of this variant using software algorithms for the prediction of the effect of nucleotide changes on splicing yielded predictions that this variant may affect proper NF1 mRNA splicing. Based on the available information, this variant is classified as likely pathogenic.

The c.6756+3A>G intronic variant results from an A to G substitution 3 nucleotides after coding exon 44 in the NF1 gene. In silico splice site analysis predicts that this alteration will weaken the native splice donor site. RNA studies have demonstrated that this alteration results in abnormal splicing in the set of samples tested (Ambry internal data). This variant was reported in individual(s) with features consistent with Neurofibromatosis type 1 (Ambry internal data). Other variant(s) impacting the same donor site (c.6756+1delG) have been shown to have a similar impact on splicing in individual(s) with features consistent with Neurofibromatosis type 1 (Ambry internal data). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD).
