Hereditary cancer-predisposing syndrome Uncertain:1Benign:2
This missense variant replaces methionine with valine at codon 1083 of the BRCA1 protein. Computational prediction suggests that this variant may not impact protein structure and function. To our knowledge, functional studies have not been reported for this variant. This variant has been reported in at least one individual affected with breast cancer (PMID: 19996028, 33471991) and an individual with personal or family history of breast, ovarian or related cancer (PMID: 35753294). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.
This alteration is classified as likely benign based on a combination of the following: seen in unaffected individuals, population frequency, intact protein function, lack of segregation with disease, co-occurrence, RNA analysis, in silico models, amino acid conservation, lack of disease association in case-control studies, and/or the mechanism of disease or impacted region is inconsistent with a known cause of pathogenicity.
Missense variant in a coldspot region where missense variants are very unlikely to be pathogenic (PMID:31911673).
Hereditary breast ovarian cancer syndrome Uncertain:2
This sequence change replaces methionine, which is neutral and non-polar, with valine, which is neutral and non-polar, at codon 1083 of the BRCA1 protein (p.Met1083Val). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with breast cancer (PMID: 19996028). This variant is also known as 3366A>G. ClinVar contains an entry for this variant (Variation ID: 185061). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt BRCA1 protein function with a negative predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
The missense variant NM_007294.4(BRCA1):c.3247A>G (p.Met1083Val) has not been reported previously as a pathogenic variant, to our knowledge. There is a small physicochemical difference between methionine and valine, which is not likely to impact secondary protein structure as these residues share similar properties. The p.Met1083Val variant is not predicted to introduce a novel splice site by any splice site algorithm. For these reasons, this variant has been classified as Uncertain Significance

Variant summary: BRCA1 c.3247A>G (p.Met1083Val) results in a conservative amino acid change in the encoded protein sequence. Four of four in-silico tools predict a benign effect of the variant on protein function. The variant was absent in 250484 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance .c.3247A>G has been reported in the literature in individuals affected with BRCA1-related cancers (e.g. Alkhalaf_2010, Oktay_2010, Dorling_2021, Bisgin_2022). However, these report(s) do not provide unequivocal conclusions about association of the variant with Hereditary Breast And Ovarian Cancer Syndrome. Co-occurrences with other pathogenic variant(s) have been reported (BRCA2 c.9097dupA, p.3033fs*11), providing supporting evidence for a benign role (Bisgin_2022). To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 35753294, 33471991, 19996028). ClinVar contains an entry for this variant (Variation ID: 185061). Based on the evidence outlined above, the variant was classified as uncertain significance.
BRCA1-related cancer predisposition Uncertain:1
This missense variant replaces methionine with valine at codon 1083 of the BRCA1 protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). Therefore, this variant is classified as a Variant of Uncertain Significance.
not provided Uncertain:1
Observed in individuals with BRCA1-related cancer (Oktay et al., 2010; Bisgin et al., 2022); Not observed at significant frequency in large population cohorts (gnomAD); In silico analysis supports that this missense variant does not alter protein structure/function; Also known as 3366A>G; This variant is associated with the following publications: (PMID: 28726806, 18092194, 32377563, 29884841, 35753294, 31131967, 33471991, 19996028)
Malignant tumor of breast Uncertain:1
The BRCA1, p.Met1083Val variant was not identified in the literature and was identified only in the ClinVar database (classified as an unclassified variant by the Ambry Genetics). The p.Met1083 residue is not conserved in mammals and computational analyses (PolyPhen-2, SIFT, AlignGVGD, BLOSUM, MutationTaster) do not suggest a high likelihood of impact to the protein. However, this information is not predictive enough to rule out pathogenicity. The variant occurs outside of the splicing consensus sequence and 1 of 5 in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer, HumanSpliceFinder) predict a greater than 10% difference in splicing; this is not very predictive of pathogenicity. In summary, based on the above information, the clinical significance of this variant cannot be determined with certainty at this time. This variant is classified as a variant of unknown significance.
