The BRIP1 c.205G>A (p.Gly69Arg ) missense change is absent in gnomAD v2.1.1 (https://gnomad.broadinstitute.org/). The in silico tool REVEL predicts a benign effect on protein function, but to our knowledge this prediction has not been confirmed by functional studies. This variant is located adjacent to the donor splice site of intron 3, and algorithms that predict the impact of sequence changes on splicing indicate that this variant does not affect splicing. In addition, internal RNA data demonstrates normal splicing. A case-control study of 13213 breast cancer patients and 5242 UK controls identified the variant only in one unaffected individual (PMID:? 2692136). The variant was also reported in individual(s) meeting criteria for hereditary breast or colon cancer testing (PMID: 25318351). To our knowledge, this variant has not been reported in individuals with Fanconi anemia. In summary, the evidence currently available is insufficient to determine the clinical significance of this variant. It has therefore been classified as of uncertain significance. -

This missense variant replaces glycine with arginine at codon 69 of the BRIP1 protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). Splice site prediction tools suggest that this variant may impact RNA splicing but this has not been demonstrated by experimental RNA studies. To our knowledge, protein functional studies have not been performed for this variant. This variant has been reported in individuals affected with breast cancer in the literature, but also in unaffected individuals (PMID: 25318351, 26921362). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). The p.G69R variant (also known as c.205G>A), located in coding exon 2 of the BRIP1 gene, results from a G to A substitution at nucleotide position 205. The amino acid change results in glycine to arginine at codon 69, an amino acid with dissimilar properties. In addition, this change occurs in the last base pair of coding exon 2, which makes it likely to have some effect on normal mRNA splicing. However, RNA studies have demonstrated that this alteration does not result in abnormal splicing in the set of samples tested (Ambry internal data). In one study, this variant was reported in one of 5242 controls but in 0/13213 breast cancer cases (Easton DF et al. J. Med. Genet., 2016 05;53:298-309). This amino acid position is poorly conserved in available vertebrate species. In addition, as a missense substitution, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

Variant summary: BRIP1 c.205G>A (p.Gly69Arg) results in a non-conservative amino acid change located in the P-loop containing nucleotide triphosphate hydrolases domain (IPR027417) of the encoded protein sequence. Three of five in-silico tools predict a benign effect of the variant on protein function. Several computational tools predict a significant impact on normal splicing: Two predict the variant weakens a 5' donor site. One predicts the variant abolishes a 5' splicing donor site. One predicts the variant creates a 3' acceptor site. Multiple laboratories have submitted RNA splicing evidence for this variant to ClinVar, without clear results. The variant was absent in 251316 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.205G>A has been reported in the presumed heterozygous state in the literature in at least 1 individual undergoing genetic testing using a cancer gene panel (example, Yorczyk_2015). These report(s) do not provide unequivocal conclusions about association of the variant with BRIP1-related conditions. This missense change has been observed in individual(s) with BRIP1-related conditions (PMID: 25318351). ClinVar contains an entry for this variant (Variation ID: 230053). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt BRIP1 protein function with a negative predictive value of 95%. Variants that disrupt the consensus splice site are a relatively common cause of aberrant splicing (PMID: 17576681, 9536098). RNA analysis performed to evaluate the impact of this missense change on mRNA splicing indicates it does not significantly alter splicing (internal data).
