Experimental studies have shown that this missense change affects NPC1 function (PMID: 16138904). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt NPC1 protein function. ClinVar contains an entry for this variant (Variation ID: 555143). This missense change has been observed in individuals with Niemann-Pick type C disease (PMID: 25349751, 26666848). This variant is not present in population databases (gnomAD no frequency). This sequence change replaces phenylalanine, which is neutral and non-polar, with leucine, which is neutral and non-polar, at codon 1087 of the NPC1 protein (p.Phe1087Leu). - |

Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 251106 control chromosomes (gnomAD v2 Exomes dataset). c.3259T>C has been reported in the literature in several compound heterozygous individuals affected with Niemann-Pick Disease Type C (e.g., Park_2003, Pina-Aguilar_2014, Imrie_2015, Rodriguez-Gil_2021). These data indicate that the variant may be associated with disease. At least two publications report experimental evidence evaluating an impact on protein function, finding that the variant was unable to revert edelfosine resistance and Western blotting revealed a severe defect in protein expression in a yeast model (Berger_2005); cholesterol accumulation was also increased approximately 10-fold in another in vitro model relative to wildtype (Qian_2020). The following publications have been ascertained in the context of this evaluation (PMID: 16138904, 26666848, 12955717, 25349751, 34296265, 32544384). Two submitters have reported clinical-significance assessments for this variant to ClinVar after 2014. All submitters classified the variant as pathogenic/likely pathogenic.
