This sequence change replaces arginine, which is basic and polar, with tryptophan, which is neutral and slightly polar, at codon 404 of the NPC1 protein (p.Arg404Trp). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with Niemann-Pick type C disease (PMID: 12955717, 26981555). ClinVar contains an entry for this variant (Variation ID: 499939). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt NPC1 protein function with a positive predictive value of 95%. This variant disrupts the p.Arg404 amino acid residue in NPC1. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 11349231, 11545687, 22065762, 26666848, 27581084). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. Another missense variant in the same residue p.R404p was identified in patient affected with Niemann-Pick disease Dubey V et al. 2020. The p.Arg404Trp variant is absent in gnomAD Exomes. This variant has been submitted to the ClinVar database as Uncertain significance / Pathogenic/ Likely Pathogenic multiple submitters. Multiple lines of computational evidence Polyphen - Probably damaging, SIFT – Damaging and Mutation Taster - Disease causing predict a damaging effect on protein structure and function for this variant. The reference amino acid on NPC1 gene is predicted as conserved by GERP++ and PhyloP across 100 vertebrates. The amino acid Arg at position 404 is changed to a Trp changing protein sequence and it might alter its composition and physico-chemical properties. Variant summary: NPC1 c.1210C>T (p.Arg404Trp) results in a non-conservative amino acid change in the encoded protein sequence. Algorithms developed to predict the effect of missense changes on protein structure and function all suggest that this variant is likely to be disruptive. The variant was absent in 251292 control chromosomes (gnomAD). c.1210C>T has been observed in individuals affected with Niemann-Pick Disease Type C (e.g., Park_2003, Reunert_2016, Girard_2024). These data indicate that the variant may be associated with disease. A different variant affecting the same codon has been classified as likely pathogenic/pathogenic by our lab (c.1211G>A, p.Arg404Gln), supporting the critical relevance of codon 404 to NPC1 protein function. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 12955717, 26981555, 39507854). ClinVar contains an entry for this variant (Variation ID: 499939).
