This sequence change replaces asparagine with threonine at codon 194 of the TNNI3 protein (p.Asn194Thr). The asparagine residue is highly conserved and there is a small physicochemical difference between asparagine and threonine. This variant is present in population databases (rs730881081, ExAC 0.001%). This missense change has been observed in individual(s) with hypertrophic cardiomyopathy (PMID: 27532257). ClinVar contains an entry for this variant (Variation ID: 519178). Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Probably Damaging"; Align-GVGD: "Class C0"). This variant disrupts the p.Asn194 amino acid residue in TNNI3. Other variant(s) that disrupt this residue have been observed in individuals with TNNI3-related conditions (PMID: 25524337, 31568572), which suggests that this may be a clinically significant amino acid residue. The p.N194T variant (also known as c.581A>C), located in coding exon 8 of the TNNI3 gene, results from an A to C substitution at nucleotide position 581. The asparagine at codon 194 is replaced by threonine, an amino acid with similar properties. This alteration has been reported in a hypertrophic cardiomyopathy (HCM) testing cohort; however, clinical details were not provided (Walsh R et al. Genet. Med., 2017 Feb;19:192-203). An alternate substitution at this amino acid position, p.N194S, has been detected in at least two patients with confirmed HCM (Coppini R et al. J. Am. Coll. Cardiol., 2014 Dec;64:2589-600; Cecconi M et al. Int. J. Mol. Med., 2016 Oct;38:1111-24). This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
