Variant summary
Our verdict is Pathogenic. The variant received 19 ACMG points: 19P and 0B. PM1PM2PM5PP2PP3_StrongPP5_Very_Strong
The NM_001165963.4(SCN1A):c.4297G>C(p.Gly1433Arg) variant causes a missense change involving the alteration of a conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. 14/22 in silico tools predict a damaging outcome for this variant. Variant has been reported in ClinVar as Pathogenic (★★). Another nucleotide change resulting in the same amino acid substitution has been previously reported as Uncertain significance in ClinVar. Another variant affecting the same amino acid position, but resulting in a different missense (i.e. G1433E) has been classified as Pathogenic.

For these reasons, this variant has been classified as Pathogenic. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt SCN1A protein function. ClinVar contains an entry for this variant (Variation ID: 206814). This missense change has been observed in individual(s) with clinical features of SCN1A-related conditions and/or Dravet syndrome (PMID: 20729507, 29655203). In at least one individual the variant was observed to be de novo. This variant is not present in population databases (gnomAD no frequency). This sequence change replaces glycine, which is neutral and non-polar, with arginine, which is basic and polar, at codon 1433 of the SCN1A protein (p.Gly1433Arg). -

p.Gly1433Arg (GGA>CGA): c.4297 G>C in exon 22 of the SCN1A gene (NM_001165963.1) The G1433R missense mutation in the SCN1A gene has been reported previously as a de novo mutation in association with severe myoclonic epilepsy of infancy (Dravet syndrome) (Nicita el at., 2010). Different missense substitutions at the same position (G1433E, G1433V) have also been reported in association with Dravet syndrome. The variant is found in EPILEPSY panel(s). -
