The c.17104+5G>A variant in the TTN gene has not been reported previously as a pathogenic variantnor as a benign variant, to our knowledge. This variant reduces the quality of the splice donor site inintron 68. However, in the absence of RNA/functional studies, the actual effect of this sequencechange in this individual is unknown. The c.17104+5G>A variant is not observed in large populationcohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). Weinterpret c.17104+5G>A as a variant of uncertain significance. -

In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Aberrant splicing can lead to protein truncation or the loss of protein expression. Truncations in this region of the TTN gene, known as the I-band, have been reported to be highly prevalent in the general population and unaffected individuals (PMID: 26701604, 22335739, 25589632). However, they have also been reported in individuals affected with autosomal recessive centronuclear myopathy (PMID: 23975875). Nucleotide substitutions within the consensus splice site are a relatively common cause of aberrant splicing (PMID: 17576681, 9536098). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site, but this prediction has not been confirmed by published transcriptional studies. This variant has not been reported in the literature in individuals with TTN-related disease. This variant is not present in population databases (ExAC no frequency). This sequence change falls in intron 71 of the TTN gene. It does not directly change the encoded amino acid sequence of the TTN protein, but it affects a nucleotide within the consensus splice site of the intron. -
