Genes affected
CTLA4 (HGNC:2505): (cytotoxic T-lymphocyte associated protein 4) This gene is a member of the immunoglobulin superfamily and encodes a protein which transmits an inhibitory signal to T cells. The protein contains a V domain, a transmembrane domain, and a cytoplasmic tail. Alternate transcriptional splice variants, encoding different isoforms, have been characterized. The membrane-bound isoform functions as a homodimer interconnected by a disulfide bond, while the soluble isoform functions as a monomer. Mutations in this gene have been associated with insulin-dependent diabetes mellitus, Graves disease, Hashimoto thyroiditis, celiac disease, systemic lupus erythematosus, thyroid-associated orbitopathy, and other autoimmune diseases. ACMG classification
Classification made for transcript
Verdict is Pathogenic. Variant got 12 ACMG points.
PVS1
Loss of function variant, product does not undergo nonsense mediated mRNA decay. Variant located near the start codon (<100nt), not predicted to undergo nonsense mediated mRNA decay. There are 33 pathogenic variants in the truncated region.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 2-203867943-A-ATGGGT is Pathogenic according to our data. Variant chr2-203867943-A-ATGGGT is described in ClinVar as [Pathogenic]. Clinvar id is 852228.Status of the report is criteria_provided_single_submitter, 1 stars.

Autoimmune lymphoproliferative syndrome due to CTLA4 haploinsufficiency Pathogenic:1
Pathogenic, criteria provided, single submitter | clinical testing | Labcorp Genetics (formerly Invitae), Labcorp | Jan 03, 2019 | For these reasons, this variant has been classified as Pathogenic. Loss-of-function variants in CTLA4 are known to be pathogenic (PMID: 25213377, 25329329). This variant has not been reported in the literature in individuals with CTLA4-related conditions. This variant is not present in population databases (ExAC no frequency). This sequence change creates a premature translational stop signal (p.Ala2Glyfs*14) in the CTLA4 gene. It is expected to result in an absent or disrupted protein product. - |
