This sequence change affects an acceptor splice site in intron 14 of the COL4A4 gene. It is expected to disrupt RNA splicing. Variants that disrupt the donor or acceptor splice site typically lead to a loss of protein function (PMID: 16199547), and loss-of-function variants in COL4A4 are known to be pathogenic (PMID: 21196518, 24854265, 25307543). This variant is present in population databases (rs375450996, gnomAD 0.002%). This variant has not been reported in the literature in individuals affected with COL4A4-related conditions. ClinVar contains an entry for this variant (Variation ID: 447195). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic.

This variant affects the canonical splice acceptor site of intron 14 and is therefore predicted to interfere with splicing and result in loss of normal protein function through either protein truncation or nonsense-mediated mRNA decay (NMD). This variant has not been previously reported or functionally characterized in the literature to our knowledge. Splice site variants in COL4A4 have been previously reported in individuals with COL4A4-related disorders (PMID: 31686460, 30968591, 22887978, 24854265). Multiple splice prediction tools suggest this variant is likely to interfere with normal splicing. Based on the available evidence, the c.871-1G>C variant is classified as Likely Pathogenic.
