MSH2 (HGNC:7325): (mutS homolog 2) This locus is frequently mutated in hereditary nonpolyposis colon cancer (HNPCC). When cloned, it was discovered to be a human homolog of the E. coli mismatch repair gene mutS, consistent with the characteristic alterations in microsatellite sequences (RER+ phenotype) found in HNPCC. This pathogenic variant is denoted MSH2 c.350 at the cDNA level and p.Trp117Ter (W117X) at the protein level. The substitution creates a nonsense variant, which changes a Tryptophan to a premature stop codon (TGG>TAG), and is predicted to cause loss of normal protein function through either protein truncation or nonsense-mediated mRNA decay. Although this variant has not, to our knowledge, been reported in the literature, it is considered pathogenic.

The p.W117* variant (also known as c.350G>A), located in coding exon 2 of the MSH2 gene, results from a G to A substitution at nucleotide position 350. This changes the amino acid from a tryptophan to a stop codon within coding exon 2. This variant was reported in individual(s) with features consistent with MSH2-related Lynch syndrome (Ambry internal data). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. Based on the supporting evidence, this variant is interpreted as a disease-causing mutation.
