Likely pathogenic, criteria provided, single submitter | clinical testing | Ambry Genetics | Aug 28, 2021 | The p.L414P variant (also known as c.1241T>C), located in coding exon 7 of the MSH2 gene, results from a T to C substitution at nucleotide position 1241. The leucine at codon 414 is replaced by proline, an amino acid with similar properties. This variant was reported in a patient diagnosed with duodenal cancer at age 33 and his mother was diagnosed with uterine cancer at age 42 as well as a retroperitoneal lymph node tumor at age 49 (Levene S et al. Fam. Cancer, 2003;2:15-25). This variant has also been identified as somatic in conjunction with a somatic pathogenic MSH2 variant in a colorectal tumor and an adenoma that both demonstrated high microsatellite instability with loss of MSH2, MSH6 expression by immunohistochemistry (Ambry internal data). In a massively parallel cell-based functional assay testing susceptibility to a DNA damaging agent, 6-thioguanine (6-TG), this variant was determined to be functionally deleterious (Jia X et al. Am J Hum Genet, 2021 01;108:163-175). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis.
