The variant was also identified in dbSNP (ID: rs63750820) as “With other allele”, ClinVar (classified as benign by GeneDx and Invitae, as likely benign by Ambry Genetics and Color Genomics, and as uncertain significance by Illumina Clinical Services), Mismatch Repair Genes Variant Database, and Insight Hereditary Tumors Database (1x). The variant was not identified in COGR, COSMIC, MutDB, UMD-LSDB, or Zhejiang Colon Cancer databases. The variant was identified in control databases in 34 of 277178 chromosomes at a frequency of 0.0001 (Genome Aggregation Database Feb 27, 2017). The variant was identified in the following populations: Ashkenazi Jewish in 29 of 10150 chromosomes (freq: 0.003), Other in 3 of 6464 chromosomes (freq: 0.0005), and European in 2 of 126680 chromosomes (freq: 0.00002), while the variant was not observed in the African, Latino, East Asian, Finnish, or South Asian populations. In an in vitro study of mismatch repair efficiency, this variant demonstrated in tact MMR activity (Tronov 2006). The p.Gly520= variant is not expected to have clinical significance because it does not result in a change of amino acid and is not located in a known consensus splice site. In addition, 2 of 5 in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer, Human Splicing Finder) predict that the variants creates a new 5’ splice site. In summary, based on the above information, the clinical significance of this variant cannot be determined with certainty at this time although we would lean towards a more benign role for this variant.
