MSH2 (HGNC:7325): (mutS homolog 2) This locus is frequently mutated in hereditary nonpolyposis colon cancer (HNPCC). When cloned, it was discovered to be a human homolog of the E. coli mismatch repair gene mutS, consistent with the characteristic alterations in microsatellite sequences (RER+ phenotype) found in HNPCC. This sequence change falls in intron 14 of the MSH2 gene. It does not directly change the encoded amino acid sequence of the MSH2 protein. It affects a nucleotide within the consensus splice site.

The c.2459-3T>G intronic variant results from a T to G substitution 3 nucleotides upstream from coding exon 15 in the MSH2 gene. This nucleotide position is not well conserved in available vertebrate species. This variant has been identified in a proband whose Lynch syndrome-associated tumor demonstrated high microsatellite instability and weak expression of MSH6 by immunohistochemistry (Meulemans L et al. J Med Genet, 2023 May;60:450-459). In the same study, the effect of the c.2459-3T>G variant on splicing was analyzed using a minigene assay, which generated three aberrant transcripts due to the weakening of the native acceptor site and the use of a downstream and upstream cryptic acceptor site (Meulemans L et al. J Med Genet, 2023 May;60:450-459). In silico splice site analysis predicts that this alteration will weaken the native splice acceptor site and will result in the creation or strengthening of a novel splice acceptor site. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
