This missense variant replaces glutamine with glutamic acid at codon 626 of the MSH6 protein. Computational prediction is inconclusive regarding the impact of this variant on protein structure and function (internally defined REVEL score threshold 0.5 < inconclusive < 0.7, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in individuals affected with pancreatic cancer but also in multiple healthy control individuals in the same study (PMID: 32980694). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
The p.Q626E variant (also known as c.1876C>G), located in coding exon 4 of the MSH6 gene, results from a C to G substitution at nucleotide position 1876. The glutamine at codon 626 is replaced by glutamic acid, an amino acid with highly similar properties. This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. In addition, the CoDP in silico tool predicts this alteration to have minor impact on molecular function, with a score of 0.018 (Terui H et al. J. Biomed. Sci. 2013 Apr;20:25). Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

Variant summary: MSH6 c.1876C>G (p.Gln626Glu) results in a conservative amino acid change located in the DNA mismatch repair protein MutS, connector domain (IPR007860) of the encoded protein sequence. Four of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 7.1e-05 in 297786 control chromosomes (gnomAD and publication), predominantly at a frequency of 0.00044 within Japanese controls who were 60 years old or over and did not have personal nor family history of cancers (Mizukami_2020). The observed variant frequency within Japanese control individuals is approximately 3-fold of the estimated maximal expected allele frequency for a pathogenic variant in MSH6 causing Lynch Syndrome phenotype (0.00014), suggesting that the variant may be a benign polymorphism found primarily in the Japanese population. c.1876C>G has been reported in the literature in Japanese individuals affected with pancreatic cancer but it was also reported in multiple controls, as described above (Mizukami_2020). This report does not provide unequivocal conclusions about association of the variant with Lynch Syndrome. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Two ClinVar submitters (evaluation after 2014) cite the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as uncertain significance until additional evidence of clinical and or functional importance becomes available. -

This sequence change replaces glutamine, which is neutral and polar, with glutamic acid, which is acidic and polar, at codon 626 of the MSH6 protein (p.Gln626Glu). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with pancreatic cancer (PMID: 32980694). ClinVar contains an entry for this variant (Variation ID: 525826). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt MSH6 protein function with a negative predictive value of 95%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
