The p.L735P variant (also known as c.2204T>C), located in coding exon 4 of the MSH6 gene, results from a T to C substitution at nucleotide position 2204. The leucine at codon 735 is replaced by proline, an amino acid with similar properties. This variant has been identified in multiple probands whose Lynch syndrome-associated tumor demonstrated loss of MSH6 expression by immunohistochemistry (Li S et al. J. Med. Genet. 2020 Jan;57:62-69; External communication). This variant was seen in 1/150 unselected patients with recurrent or metastatic prostate cancer (Isaacsson Velho P et al. Prostate, 2018 Apr;78:401-407). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the majority of available evidence to date, this variant is likely to be pathogenic.
