The glycine at codon 185 is replaced by arginine, an amino acid with dissimilar properties. This variant has been observed in at least one individual with a personal and/or family history of pheochromocytoma (Ambry internal data). This variant has been detected in a patient with bilateral adrenal pheochromocytomas and extra-adrenal retroperitoneal paraganglioma (Neumann HP et al. J Clin Endocrinol Metab, 2011 Aug;96:E1279-82). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). Based on the majority of available evidence to date, this variant is likely to be pathogenic. - |

The glycine residue is highly conserved and there is a moderate physicochemical difference between glycine and arginine. In summary, this variant is a rare missense change with uncertain impact on protein function. While it is absent from the population and reported in an affected individual, the available evidence is currently insufficient to determine its role in disease. Therefore, it has been classified as a Variant of Uncertain Significance. This variant has been reported Β¬β in the literature in an individual affected with extraadrenal retroperitoneal and bilateral adrenal tumors (PMID:21613359). Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies.
