The p.I371T variant (also known as c.1112T>C), located in coding exon 7 of the MYLK2 gene, results from a T to C substitution at nucleotide position 1112. The isoleucine at codon 371 is replaced by threonine, an amino acid with similar properties. This amino acid position is conserved. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this variant remains unclear.
The p.Ile371Thr variant MYLK2 has not been previously reported in individuals wi th cardiomyopathy, but has been identified in 2/8766 East Asian chromosomes by t he Exome Aggregation Consortium (ExAC; http://exac.broadinstitute.org). Computat ional prediction tools and conservation analysis do not provide strong support f or or against an impact to the protein. In summary, the clinical significance of the p.Ile371Thr variant is uncertain.

This sequence change replaces isoleucine, which is neutral and non-polar, with threonine, which is neutral and polar, at codon 371 of the MYLK2 protein (p.Ile371Thr). This variant is present in population databases (rs727505336, gnomAD 0.06%), and has an allele count higher than expected for a pathogenic variant. This variant has not been reported in the literature in individuals affected with MYLK2-related conditions. ClinVar contains an entry for this variant (Variation ID: 180083). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt MYLK2 protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
