Variant summary
Our verdict is Likely benign. The variant received -4 ACMG points: 2P and 6B. PM2BP4_StrongBP6BP7
The NM_020433.5(JPH2):c.1464C>A(p.Ala488Ala) variant causes a synonymous change involving the alteration of a non-conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a benign outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Synonymous variant affecting the same amino acid position (i.e. A488A) has been classified as Likely benign.

Submissions by phenotype
not provided Uncertain:1
c.1464C>A (silent) in the JPH2 gene (NM_020433.4) The lab classifies this variant as a variant of uncertain significance (VUS). Given a lack a case data, lack of segregation and the fact that this is a silent change that doesn't affect the sequence of the protein, we consider this variant of uncertain significance and we do not feel it is suitable for assessing risk in healthy relatives ("predictive genetic testing"). This variant has not been identified in other individuals with cardiomyopathy. This variant occurs toward the end of the protein and not in one of the eight conserved MORN repeats that occur earlier in the gene. This variant is not present in exac. -
Hypertrophic cardiomyopathy Uncertain:1
This sequence change replaces alanine, which is neutral and non-polar, with alanine, which is neutral and non-polar, at codon 488 of the JPH2 protein (Silent). This variant is not present in population databases (gnomAD no frequency). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant is not likely to affect RNA splicing. ClinVar contains an entry for this variant (Variation ID: 403851). This variant has not been reported in the literature in individuals affected with JPH2-related conditions. -
Cardiovascular phenotype Benign:1
This alteration is classified as likely benign based on a combination of the following: seen in unaffected individuals, population frequency, intact protein function, lack of segregation with disease, co-occurrence, RNA analysis, in silico models, amino acid conservation, lack of disease association in case-control studies, and/or the mechanism of disease or impacted region is inconsistent with a known cause of pathogenicity. -
