Genes affected
COL6A1 (HGNC:2211): (collagen type VI alpha 1 chain) The collagens are a superfamily of proteins that play a role in maintaining the integrity of various tissues. Collagens are extracellular matrix proteins and have a triple-helical domain as their common structural element. Collagen VI is a major structural component of microfibrils. The basic structural unit of collagen VI is a heterotrimer of the alpha1(VI), alpha2(VI), and alpha3(VI) chains. The alpha2(VI) and alpha3(VI) chains are encoded by the COL6A2 and COL6A3 genes, respectively. The protein encoded by this gene is the alpha 1 subunit of type VI collagen (alpha1(VI) chain). Mutations in the genes that code for the collagen VI subunits result in the autosomal dominant disorder, Bethlem myopathy. [provided by RefSeq, Jul 2008]

ACMG classification
Classification made for transcript
Verdict is Pathogenic. Variant got 12 ACMG points.
PVS1
Splicing +-2 bp (donor or acceptor) variant, product NOT destroyed by NMD, known LOF gene, truncates exone, which is 0.008422417 fraction of the gene. Cryptic splice site detected, with MaxEntScore 5.5, offset of 8, new splice context is: ctgtctccgcacggctccAGggg. Cryptic site results in frameshift change. If cryptic site found is not functional and variant results in exon loss, it results in inframe change.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 21-45990257-G-C is Pathogenic according to our data. Variant chr21-45990257-G-C is described in ClinVar as [Pathogenic]. Clinvar id is 290490.Status of the report is criteria_provided_multiple_submitters_no_conflicts, 2 stars. Variant chr21-45990257-G-C is described in Lovd as [Pathogenic].

Bethlem myopathy 1A Pathogenic:1
Pathogenic, criteria provided, single submitter | clinical testing | Labcorp Genetics (formerly Invitae), Labcorp | Dec 03, 2018 | For these reasons, this variant has been classified as Pathogenic. Donor and acceptor splice site variants typically lead to a loss of protein function (PMID: 16199547), and loss-of-function variants in COL6A1 are known to be pathogenic (PMID: 21280092, 20976770). In addition, donor and acceptor splice site variants in COL6A1 that result in in-frame exon skipping have also been reported to cause autosomal dominant COL6A1-related conditions (PMID: 18366090). Disruption of this splice site has been observed in individuals with Bethlem myopathy and to segregate with Bethlem myopathy in a family (PMID: 28688748, 9580662). ClinVar contains an entry for this variant (Variation ID: 290490). This variant is not present in population databases (ExAC no frequency). This sequence change affects an acceptor splice site in intron 11 of the COL6A1 gene. It is expected to disrupt RNA splicing and likely results in an absent or disrupted protein product. - |
