This sequence change replaces glutamic acid, which is acidic and polar, with lysine, which is basic and polar, at codon 509 of the LARGE1 protein (p.Glu509Lys). This variant is present in population databases (rs121908675, gnomAD 0.003%). This missense change has been observed in individual(s) with muscular dystrophy-dystroglycanopathy (PMID: 12966029, 24709677). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 6216). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt LARGE1 protein function with a positive predictive value of 80%. Experimental studies have shown that this missense change affects LARGE1 function (PMID: 15661757). For these reasons, this variant has been classified as Pathogenic.

Variant summary: LARGE1 c.1525G>A (p.Glu509Lys) results in a conservative amino acid change in the encoded protein sequence. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change. The variant allele was found at a frequency of 8e-06 in 250922 control chromosomes. c.1525G>A has been observed in compound heterozygous individuals affected with LARGE1-Related Disorders (Longman_2003, Meilleur_2014). These data indicate that the variant may be associated with disease. At least one publication reports experimental evidence evaluating an impact on protein function. The most pronounced variant effect results in mislocalization of the mutant protein and failure of mutant protein to glycosylate alpha-dystroglycan (Brockington_2005). The following publications have been ascertained in the context of this evaluation (PMID: 15661757, 12966029, 24709677). ClinVar contains an entry for this variant (Variation ID: 6216).
