Genes affected
ARSA (HGNC:713): (arylsulfatase A) The protein encoded by this gene hydrolyzes cerebroside sulfate to cerebroside and sulfate. Defects in this gene lead to metachromatic leucodystrophy (MLD), a progressive demyelination disease which results in a variety of neurological symptoms and ultimately death. Alternatively spliced transcript variants have been described for this gene. [provided by RefSeq, Dec 2010]

ACMG classification
Classification made for transcript
Verdict is Likely_pathogenic. Variant got 8 ACMG points.
PVS1
Loss of function variant, product does not undergo nonsense mediated mRNA decay. Variant is located in the 3'-most exon, not predicted to undergo nonsense mediated mRNA decay. There are 5 pathogenic variants in the truncated region.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 22-50625228-C-A is Pathogenic according to our data. Variant chr22-50625228-C-A is described in ClinVar as [Pathogenic]. Clinvar id is 941799.Status of the report is criteria_provided_single_submitter, 1 stars.

Pathogenic, criteria provided, single submitter | clinical testing | Labcorp Genetics (formerly Invitae), Labcorp | Jul 26, 2023 | For these reasons, this variant has been classified as Pathogenic. This sequence change creates a premature translational stop signal (p.Glu483*) in the ARSA gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 27 amino acid(s) of the ARSA protein. This variant is present in population databases (rs148352371, gnomAD 0.009%). This variant has not been reported in the literature in individuals affected with ARSA-related conditions. ClinVar contains an entry for this variant (Variation ID: 941799). This variant disrupts a region of the ARSA protein in which other variant(s) (p.Cys495Phe) have been determined to be pathogenic (PMID: 19021637). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. - |
