Genes affected
VHL (HGNC:12687): (von Hippel-Lindau tumor suppressor) This gene encodes a component of a ubiquitination complex. The encoded protein is involved in the ubiquitination and degradation of hypoxia-inducible-factor (HIF), which is a transcription factor that plays a central role in the regulation of gene expression by oxygen. In addition to oxygen-related gene expression, this protein plays a role in many other cellular processes including cilia formation, cytokine signaling, regulation of senescence, and formation of the extracellular matrix. Variants of this gene are associated with von Hippel-Lindau syndrome, pheochromocytoma, erythrocytosis, renal cell carcinoma, and cerebellar hemangioblastoma. [provided by RefSeq, Jun 2022]

Uncertain significance, criteria provided, single submitterclinical testingLabcorp Genetics (formerly Invitae), LabcorpSep 25, 2021In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. This variant disrupts the p.Arg79 amino acid residue in VHL. Other variant(s) that disrupt this residue have been observed in individuals with VHL-related conditions (PMID: 15642680, 24555745), which suggests that this may be a clinically significant amino acid residue. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Tolerated"; PolyPhen-2: "Possibly Damaging"; Align-GVGD: "Class C0"). ClinVar contains an entry for this variant (Variation ID: 411988). This variant has not been reported in the literature in individuals affected with VHL-related conditions. This variant is not present in population databases (ExAC no frequency). This sequence change replaces arginine with glycine at codon 79 of the VHL protein (p.Arg79Gly). The arginine residue is highly conserved and there is a moderate physicochemical difference between arginine and glycine. -

Uncertain significance, criteria provided, single submitterclinical testingGeneDxJul 03, 2023Not observed at significant frequency in large population cohorts (gnomAD); In silico analysis supports that this missense variant has a deleterious effect on protein structure/function; Observed in an individual with retinal hemangioblastoma and in another individual with unspecified personal and family history of VHL-related cancers (Reich et al., 2021); This variant is associated with the following publications: (PMID: 33720516) -

Uncertain significance, criteria provided, single submitterclinical testingAmbry GeneticsMar 06, 2024The p.R79G variant (also known as c.235C>G), located in coding exon 1 of the VHL gene, results from a C to G substitution at nucleotide position 235. The arginine at codon 79 is replaced by glycine, an amino acid with dissimilar properties. This alteration has been reported in a Japanese individual with paraganglioma or pheochromocytoma (Yonamine M et al. Cancers (Basel), 2021 Aug;13:). This alteration has also been reported in a cohort of individuals with clinical suspicion of von Hippel-Lindau disease (VHL) (Reich M et al. Acta Ophthalmol, 2021 Dec;99:e1492-e1500). This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the available evidence, the clinical significance of this alteration remains unclear. -
