This missense variant replaces serine with cysteine at codon 139 of the VHL protein. Computational prediction suggests that this variant may have deleterious impact on protein structure and function (internally defined REVEL score threshold >= 0.7, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has not been reported in individuals affected with VHL-related disorders in the literature. This variant has been identified in 23/282886 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

Observed with a deletion of VHL exon 3 in a child with clinical features of familial erythrocytosis type 2 (PMID: 33938902); In silico analysis indicates that this missense variant does not alter protein structure/function; This variant is associated with the following publications: (PMID: 25078357, Barron2019[abstract], 33938902) -

The p.S139C variant (also known as c.416C>G), located in coding exon 2 of the VHL gene, results from a C to G substitution at nucleotide position 416. The serine at codon 139 is replaced by cysteine, an amino acid with dissimilar properties. Based on data from gnomAD, the frequency for this variant is above the maximum credible frequency for a VHL-causing variant based on internally established thresholds (Karczewski et al. Nature. 2020 May;581(7809):434-443; Whiffin et al. Genet Med. 2017 10;19:1151-1158). However, this variant was identified in conjunction with a pathogenic VHL deletion in a child with features suggestive of Chuvash polycythemia (Núñez-Martínez PM et al. Bol Med Hosp Infant Mex, 2021 May). This amino acid position is not well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this variant remains unclear. -

Von Hippel-Lindau syndrome;C1837915:Chuvash polycythemia Uncertain:1
This sequence change replaces serine, which is neutral and polar, with cysteine, which is neutral and slightly polar, at codon 139 of the VHL protein (p.Ser139Cys). This variant is present in population databases (rs587780732, gnomAD 0.07%). This variant has not been reported in the literature in individuals affected with VHL-related conditions. ClinVar contains an entry for this variant (Variation ID: 135956). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt VHL protein function with a positive predictive value of 95%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
