The p.H463Q variant (also known as c.1389C>A), located in coding exon 4 of the CASR gene, results from a C to A substitution at nucleotide position 1389. The histidine at codon 463 is replaced by glutamine, an amino acid with highly similar properties. This variant has been detected in two individuals from familial hypocalciuric hypercalcemia cohorts; however, details were limited (Nissen PH et al. Clin. Chim. Acta, 2012 Mar;413:605-11; Carlsson ER et al. Case Rep Endocrinol, 2019 Feb;2019:9468252). This amino acid position is not well conserved in available vertebrate species, and glutamine is the reference amino acid in other vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.

This variant is present in population databases (rs201572629, gnomAD 0.006%). This missense change has been observed in individual(s) with clinical features of CASR-related conditions (PMID: 22192860). ClinVar contains an entry for this variant (Variation ID: 573748). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be tolerated. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
