Defects in this gene are the cause of familial arrhythmogenic right ventricular dysplasia type 5 (ARVD5), also known as arrhythmogenic right ventricular cardiomyopathy type 5 (ARVC5). Arrhythmogenic right ventricular dysplasia is an inherited disorder, often involving both ventricles, and is characterized by ventricular tachycardia, heart failure, sudden cardiac death, and fibrofatty replacement of cardiomyocytes. This gene contains a response element for PPAR gamma (an adipogenic transcription factor), which may explain the fibrofatty replacement of the myocardium, a characteristic pathological finding in ARVC. The p.G289R variant (also known as c.865G>A), located in coding exon 10 of the TMEM43 gene, results from a G to A substitution at nucleotide position 865. The glycine at codon 289 is replaced by arginine, an amino acid with dissimilar properties. This variant has been reported in stillbirth, arrhythmogenic right ventricular cardiomyopathy (ARVC) and cardiomyopathy cohorts (Lin CY et al. J Cardiovasc Electrophysiol, 2019 Apr;30:582-592; Sahlin E et al. PLoS One, 2019 Jan;14:e0210017; Zhu Y et al. Eur J Med Genet, 2022 Mar;65:104429). This amino acid position is well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this variant remains unclear. -
