MLH1 (HGNC:7127): (mutL homolog 1) The protein encoded by this gene can heterodimerize with mismatch repair endonuclease PMS2 to form MutL alpha, part of the DNA mismatch repair system. When MutL alpha is bound by MutS beta and some accessory proteins, the PMS2 subunit of MutL alpha introduces a single-strand break near DNA mismatches, providing an entry point for exonuclease degradation. The encoded protein is also involved in DNA damage signaling and can heterodimerize with DNA mismatch repair protein MLH3 to form MutL gamma, which is involved in meiosis. This gene was identified as a locus frequently mutated in hereditary nonpolyposis colon cancer (HNPCC). [provided by RefSeq, Aug 2017]

This variant has shown segregation with MLH1-associated cancers in a family (communication with an external laboratory; ClinVar SCV000543593). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). Although there is a suspicion that this variant may have a pathogenic role, additional evidence is necessary to determine the role of this variant in disease conclusively. The glycine at codon 98 is replaced by serine, an amino acid with similar properties. This variant has been identified in individuals meeting Amsterdam criteria and/or with tumors demonstrating high microsatellite instability (Tournier I et al. Hum. Mutat. 2008 Dec;29:1412-24; Borras E et al. Cancer Prev Res (Phila), 2017 Oct;10:580-587; Ambry internal data). Based on an internal structural analysis, this alteration blocks the ATP binding site (Ban C et al. Cell, 1999 Apr;97:85-97; Wu H et al. Acta Crystallogr F Struct Biol Commun, 2015 Aug;71:981-5). This variant was not reported in population-based cohorts in the Genome Aggregation Database (gnomAD). This amino acid position is highly conserved in available vertebrate species.
