This variant has a maximum subpopulation frequency of 0.01% in gnomAD v2.1.1 (https://gnomad.broadinstitute.org). To our knowledge, this variant has not been reported in individuals with Lynch syndrome or constitutional mismatch repair deficiency syndrome. In summary, the evidence currently available is insufficient to determine the clinical significance of this variant. It has therefore been classified as of uncertain significance. -

DNA sequence analysis of the MLH1 gene demonstrated a 3 base pair deletion in exon 12, c.1091_1093del. This in-frame deletion is predicted to result in the deletion of a 1 amino acid residue, p.Thr364del. This sequence change has been described in gnomAD with a frequency of 0.012 % in the African/American sub-population (dbSNP rs1381421781). This sequence change does not appear to have been previously described in patients with MLH1-related disorders. The functional significance of this sequence change is not known at present and its contribution to this individual’s disease phenotype cannot definitively be determined. -
Variant summary: MLH1 c.1091_1093delCAA (p.Thr364del) results in an in-frame deletion that is predicted to remove one amino acid from the encoded protein. The variant was absent in 250934 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. To our knowledge, no occurrence of c.1091_1093delCAA in individuals affected with Lynch Syndrome and no experimental evidence demonstrating its impact on protein function have been reported. Four clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. All laboratories classified the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as uncertain significance. -

This results in the in-frame deletion of a threonine at codon 364. This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be neutral by in silico analysis (Choi Y et al. PLoS ONE. 2012; 7(10):e46688). Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
