SCN5A (HGNC:10593): (sodium voltage-gated channel alpha subunit 5) The protein encoded by this gene is an integral membrane protein and tetrodotoxin-resistant voltage-gated sodium channel subunit. This protein is found primarily in cardiac muscle and is responsible for the initial upstroke of the action potential in an electrocardiogram. Defects in this gene have been associated with long QT syndrome type 3 (LQT3), atrial fibrillation, cardiomyopathy, and Brugada syndrome 1, all autosomal dominant cardiac diseases. This sequence change replaces arginine, which is basic and polar, with glutamine, which is neutral and polar, at codon 988 of the SCN5A protein (p.Arg988Gln). This variant is present in population databases (rs759584454, gnomAD 0.02%). This missense change has been observed in individuals with Brugada syndrome or dilated cardiomyopathy (PMID: 28341781; internal data). ClinVar contains an entry for this variant (Variation ID: 201551). An algorithm developed to predict the effect of missense changes on protein structure and function outputs the following: PolyPhen-2: "Benign". The glutamine amino acid residue is found in multiple mammalian species, which suggests that this missense change does not adversely affect protein function. This variant disrupts the p.Arg988 amino acid residue in SCN5A. Other variant(s) that disrupt this residue have been observed in individuals with SCN5A-related conditions (PMID: 23321620), which suggests that this may be a clinically significant amino acid residue. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.

The p.R988Q variant (also known as c.2963G>A), located in coding exon 16 of the SCN5A gene, results from a G to A substitution at nucleotide position 2963. The arginine at codon 988 is replaced by glutamine, an amino acid with highly similar properties. This variant has been detected in cohorts of patients reported to have Brugada syndrome or Long QT syndrome; however, clinical details were limited (G&oacute;mez J et al. J Cardiovasc Transl Res, 2014 Feb;7:133-7; Yamagata K et al. Circulation, 2017 Jun;135:2255-2270; Walsh R et al. Genet Med, 2021 Jan;23:47-58). This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
