BBS7 (HGNC:18758): (Bardet-Biedl syndrome 7) This gene encodes one of eight proteins that form the BBSome complex containing BBS1, BBS2, BBS4, BBS5, BBS7, BBS8, BBS9 and BBIP10. The BBSome complex is believed to recruit Rab8(GTP) to the primary cilium and promote ciliogenesis. The BBSome complex assembly is mediated by a complex composed of three chaperonin-like BBS proteins (BBS6, BBS10, and BBS12) and CCT/TRiC family chaperonins. Mutations in this gene are implicated in Bardet-Biedl syndrome, a genetic disorder whose symptoms include obesity, retinal degeneration, polydactyly and nephropathy; however, mutations in this gene and the BBS8 gene are thought to play a minor role and mutations in chaperonin-like BBS genes are found to be a major contributor to disease development in a multiethnic Bardet-Biedl syndrome patient population. Two transcript variants encoding distinct isoforms have been identified for this gene.[provided by RefSeq, Oct 2014]

This sequence change replaces histidine, which is basic and polar, with arginine, which is basic and polar, at codon 323 of the BBS7 protein (p.His323Arg). This variant is present in population databases (rs119466001, gnomAD 0.008%). This missense change has been observed in individuals with Bardet-Biedl syndrome (PMID: 12567324, 32531858). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 3015). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt BBS7 protein function with a negative predictive value of 80%. Experimental studies have shown that this missense change affects BBS7 function (PMID: 20498079, 22302990). For these reasons, this variant has been classified as Pathogenic. -

Variant summary: BBS7 c.968A>G (p.His323Arg) results in a non-conservative amino acid change in the encoded protein sequence. Algorithms developed to predict the effect of missense changes on protein structure and function all suggest that this variant is likely to be disruptive. The variant allele was found at a frequency of 2.8e-05 in 251292 control chromosomes. c.968A>G has been observed in multiple homozygous individuals affected with Bardet-Biedl Syndrome (Badano_2003). These data indicate that the variant is very likely to be associated with disease. At least one publication reports experimental evidence evaluating an impact on protein function, and the results show that this variant has a dominant negative effect on normal protein function (Zaghloul_2010). The following publications have been ascertained in the context of this evaluation (PMID: 12567324, 20498079). ClinVar contains an entry for this variant (Variation ID: 3015). Based on the evidence outlined above, the variant was classified as pathogenic. -
