This alteration results from a G to A substitution at nucleotide position 643, causing the glutamic acid (E) at amino acid position 215 to be replaced by a lysine (K). Based on data from gnomAD, the A allele has an overall frequency of 0.007% (19/282798) total alleles studied. The highest observed frequency was 0.058% (6/10366) of Ashkenazi Jewish alleles. This alteration has been reported in a dilated cardiomyopathy cohort and an idiopathic ventricular tachycardia cohort; however, clinical details were limited in both cases and additional alterations in other cardiac-related genes were identified in one case (Verdonschot, 2020; Guelly, 2021). This amino acid position is highly conserved in available vertebrate species. The in silico prediction for this alteration is inconclusive. Based on insufficient or conflicting evidence, the clinical significance of this alteration remains unclear. -
