Genes affected
DSP (HGNC:3052): (desmoplakin) This gene encodes a protein that anchors intermediate filaments to desmosomal plaques and forms an obligate component of functional desmosomes. Mutations in this gene are the cause of several cardiomyopathies and keratodermas, including skin fragility-woolly hair syndrome. Alternative splicing results in multiple transcript variants. Uncertain significance, criteria provided, single submitterclinical testingAll of Us Research Program, National Institutes of HealthOct 27, 2023This missense variant replaces glutamic acid with aspartic acid at codon 1304 of the DSP protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in an individual affected with unexplained sudden cardiac arrest (PMID: 29884292). This variant has been identified in 4/282178 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

Cardiovascular phenotype Uncertain:1
Uncertain significance, criteria provided, single submitterclinical testingAmbry GeneticsSep 14, 2023The p.E1304D variant (also known as c.3912G>C), located in coding exon 23 of the DSP gene, results from a G to C substitution at nucleotide position 3912. The glutamic acid at codon 1304 is replaced by aspartic acid, an amino acid with highly similar properties. This variant has been detected in an individual with sudden cardiac arrest; however, additional details were limited (Giudicessi JR et al. Int J Cardiol, 2018 Nov;270:214-220). This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
