Genes affected
CFTR (HGNC:1884): (CF transmembrane conductance regulator) This gene encodes a member of the ATP-binding cassette (ABC) transporter superfamily. The encoded protein functions as a chloride channel, making it unique among members of this protein family, and controls ion and water secretion and absorption in epithelial tissues. Channel activation is mediated by cycles of regulatory domain phosphorylation, ATP-binding by the nucleotide-binding domains, and ATP hydrolysis. Mutations in this gene cause cystic fibrosis, the most common lethal genetic disorder in populations of Northern European descent. The most frequently occurring mutation in cystic fibrosis, DeltaF508, results in impaired folding and trafficking of the encoded protein. Multiple pseudogenes have been identified in the human genome. [provided by RefSeq, Aug 2017]

This CFTR variant has been previously identified in two patients with features of cystic fibrosis2. c.1486T>C is absent from a large population database and a single submitter in ClinVar classifies it as a variant of uncertain clinical significance. The functional consequence of this variant has not been assessed, to our knowledge. Three bioinformatic tools queried predict that this substitution would probably be damaging, and the tryptophan residue at this position is evolutionarily conserved across all species assessed. Due to insufficient evidence that this variant is deleterious, we consider the clinical significance of c.1486T>C to be uncertain at this time. -

This sequence change replaces tryptophan, which is neutral and slightly polar, with arginine, which is basic and polar, at codon 496 of the CFTR protein (p.Trp496Arg). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt CFTR protein function. ClinVar contains an entry for this variant (Variation ID: 439055). This missense change has been observed in individual(s) with cystic fibrosis or clinical features of this condition (PMID: 30763667, 32227567; Invitae). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. This variant is not present in population databases (gnomAD no frequency). -

Variant summary: CFTR c.1486T>C (p.Trp496Arg) results in a non-conservative amino acid change located in the ABC transporter-like, ATP-binding domain (IPR03439) of the encoded protein sequence. Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 251324 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.1486T>C has been reported in the literature in at least one compound heterozygous individual affected with Cystic Fibrosis (Ruiz-Cabezas_2019, Janushka_2019, Januska_2023). These data do not allow any conclusion about variant significance. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. ClinVar contains an entry for this variant (Variation ID: 439055). Based on the evidence outlined above, the variant was classified as uncertain significance. -
