CFTR (HGNC:1884): (CF transmembrane conductance regulator) This gene encodes a member of the ATP-binding cassette (ABC) transporter superfamily. The encoded protein functions as a chloride channel, making it unique among members of this protein family, and controls ion and water secretion and absorption in epithelial tissues. Channel activation is mediated by cycles of regulatory domain phosphorylation, ATP-binding by the nucleotide-binding domains, and ATP hydrolysis. Mutations in this gene cause cystic fibrosis, the most common lethal genetic disorder in populations of Northern European descent. The most frequently occurring mutation in cystic fibrosis, DeltaF508, results in impaired folding and trafficking of the encoded protein. The p.Q1412* pathogenic mutation (also known as c.4234C>T), located in coding exon 26 of the CFTR gene, results from a C to T substitution at nucleotide position 4234. This changes the amino acid from a glutamine to a stop codon within coding exon 26. This alteration occurs at the 3' terminus of the CFTR gene, is not expected to trigger nonsense-mediated mRNA decay, and only impacts the last 4% of the protein. However, premature stop codons are typically deleterious in nature and the impacted region is critical for protein function (Ambry internal data). This alteration has been observed in individuals with cystic fibrosis or CFTR-related disorders (Hughes DJ et al. Hum Mutat, 1996;8:340-7; Sosnay PR et al. Nat Genet, 2013 Oct;45:1160-7; Zitkiewicz E et al. PLoS One, 2014 Feb;9:e89094). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). Based on the supporting evidence, this alteration is interpreted as a disease-causing mutation. -

This premature translational stop signal has been observed in individual(s) with cystic fibrosis (PMID: 8956039, 23974870, 24586523). ClinVar contains an entry for this variant (Variation ID: 53926). This variant disrupts a region of the CFTR protein in which other variant(s) (p.Glu1418Argfs*14) have been determined to be pathogenic (PMID: 7691344, 15638824, 23974870). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic. This variant is not present in population databases (gnomAD no frequency). This sequence change creates a premature translational stop signal (p.Gln1412*) in the CFTR gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 69 amino acid(s) of the CFTR protein. -

Variant summary: CFTR c.4234C>T (p.Gln1412X) results in a premature termination codon, predicted to cause a truncation of the encoded protein, which is a commonly known mechanism for disease. Truncations downstream of this position have been classified as pathogenic by our laboratory. The variant allele was found at a frequency of 4e-06 in 250742 control chromosomes (gnomAD). c.4234C>T has been reported in the literature in individuals affected with Cystic Fibrosis, including cases where it has been confirmed to be in trans with a pathogenic variant (e.g. Hughes_1996, Elahi_2006, Zietkiewicz_2013). These data indicate that the variant is likely to be associated with disease. At least one publication reports experimental evidence evaluating an impact on protein function (e.g. Sharma_2018). The results showed that although this variant does not cause nonsense mediated decay, it produces a truncated immature protein with little to no activity, <1% of normal. Five submitters, including the expert panel CFTR2, have provided assessments for this variant to ClinVar after 2014 and all classified the variant as pathogenic. Based on the evidence outlined above, the variant was classified as pathogenic. -
