CFTR (HGNC:1884): (CF transmembrane conductance regulator) This gene encodes a member of the ATP-binding cassette (ABC) transporter superfamily. The encoded protein functions as a chloride channel, making it unique among members of this protein family, and controls ion and water secretion and absorption in epithelial tissues. Channel activation is mediated by cycles of regulatory domain phosphorylation, ATP-binding by the nucleotide-binding domains, and ATP hydrolysis. Mutations in this gene cause cystic fibrosis, the most common lethal genetic disorder in populations of Northern European descent. The most frequently occurring mutation in cystic fibrosis, DeltaF508, results in impaired folding and trafficking of the encoded protein. This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. ClinVar contains an entry for this variant (Variation ID: 558578). This variant has not been reported in the literature in individuals affected with CFTR-related conditions. Variants downstream of this position have been classified as pathogenic by our laboratory and others in ClinVar. The variant was absent in 250854 control chromosomes (gnomAD). c.4297G>T has been reported in the literature as a compound heterozygous genotype in at least one individual who was affected with CBAVD (e.g. Qu_2023). These data suggest the variant is likely to be associated with Cystic Fibrosis/CFTR-related disorders. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publication has been ascertained in the context of this evaluation (PMID: 36604502). ClinVar contains an entry for this variant (Variation ID: 558578). Based on the evidence outlined above, the variant was classified as pathogenic. -
