The glycine at codon 924 is replaced by tryptophan, an amino acid with highly dissimilar properties, and is located in the C-terminal region. In one study, this alteration was reported in an individual with a QTc interval <500ms without known arrhythmia (Van Driest SL et al. JAMA. 2016;315(1):47-57). Other alterations affecting this amino acid position (p.G924A, c.2771G>C and p.G924E, c.2771G>A) have been previously reported in a study of long QT syndrome genetic testing, or in a patient with ventricular fibrillation (Kapplinger JD et al. Heart Rhythm. 2009;6(9):1297-303; Yap J et al. Singapore Med J. 2013;54(1):e1-4). This amino acid position is well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. - |
