Four copies of this protein interact with one copy of the KCNE2 protein to form a functional potassium channel. Mutations in this gene can cause long QT syndrome type 2 (LQT2). The Ile787Asn variant in the KCNH2 gene has not been reported as a disease-causing mutation or as a benign polymorphism to our knowledge. Ile787Asn results in a semi-conservative amino acid substitution of a non-polar Isoleucine with a polar Asparagine at a position that is well conserved across species. Consequently, in silico analysis predicts Ile787Asn is damaging to the protein structure/function. Mutations in nearby residues (Arg784Trp, Gly785Ala, Glu788Asp, Arg791Trp) have been reported in association with LQTS, further supporting the functional importance of this region of the protein. Furthermore, the Ile787Asn variant was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations.In summary, Ile787Asn in the KCNH2 gene is a good candidate for a disease-causing mutation. The variant is found in LQT panel(s). -
