This sequence change falls in intron 5 of the PMS2 gene. It does not directly change the encoded amino acid sequence of the PMS2 protein. It affects a nucleotide within the consensus splice site. This variant is not present in population databases (gnomAD no frequency). This variant has been observed in individual(s) with small bowel cancer (PMID: 23709753). ClinVar contains an entry for this variant (Variation ID: 480359). Variants that disrupt the consensus splice site are a relatively common cause of aberrant splicing (PMID: 17576681, 9536098). Studies have shown that this variant is associated with altered splicing resulting in multiple RNA products (PMID: 23709753, 26247049; Invitae). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
Hereditary cancer-predisposing syndrome Uncertain:1
The c.538-3C>G intronic variant results from a C to G substitution 3 nucleotides upstream from coding exon 6 in the PMS2 gene. This alteration has been reported in a female with small bowel cancer at age 67 years, whose tumor showed loss of MSH6/PMS2 on immunohistochemistry (Borràs E et al. J. Med. Genet., 2013 Aug;50:552-63). Functional analyses using both minigene assays and patient mRNA have shown that this alteration leads to potential aberrant transcripts of the PMS2 protein, bit it is unclear what effect this may have on the final PMS2 protein product, and, if any, the contribution to disease (Borràs E et al. J. Med. Genet., 2013 Aug;50:552-63; van der Klift HM et al. Mol Genet Genomic Med, 2015 Jul;3:327-45). This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. This nucleotide position is well conserved in available vertebrate species. Using the BDGP and ESEfinder splice site prediction tools, this alteration is predicted to abolish the native splice acceptor site and create a new cryptic acceptor site 2 nucleotides upstream; however, direct evidence is unavailable. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
